Purpose of Data Collection

● Patient’s CT Scans: needed to evaluate the case and process most services ● Patient’s Name: Name has to match the CT Scans to simply ensure the case is being processed for the correct patient ● Patient’s Date of Birth and Gender: The details have to match the CT Scans to simply ensure the case is being processed for the correct patient. Specifically, radiology report services and evaluations require the patient’s gender, date of birth and any relevant patient’s history as it is a key factor towards getting accurate results. ● Patient’s Impressions/Models/Digital Models: needed when requesting a service related to surgical guides design and/or restorations and/or ortho related cases ● Client’s Information: 1. Name, Phone Number and Address: For communication and identity verification. Address is used to ship orders that have physical components. 2. Credit Card Information: for billing.
