Purpose of Data Collection
The purpose of the data collection and processing is to support implementation and sustainability of the Treatment Foster Care Oregon (TFCO) program, including therapist training, feedback reports, performance monitoring and other aspects of quality assurance. The following types of Personal Data which all are considered as Special categories of Personal Data: • Outcomes for youth receiving TFCO. • Frequency of therapy components for youth receiving TFCO. • Behavioral components of youth receiving TFCO (Parent Daily Report (PDR), school cards, point and level system and privilege chart for three consecutive weeks). • Frequency of foster parent meetings. • Video of 3 consecutive foster parent meetings. • Frequency of clinical team meetings. • Video of 3 consecutive clinical team meetings. • Job description and supervisory structure of TFCO program staff. • A description of previous model training of TFCO program staff, and a plan for ongoing training. The categories of data subjects to whom the Personal Data relates Patients, family members and employees.
